Responsibilities/ Job Duties:
- Serve as a dedicated care coordinator for enrolled members, ensuring they receive personalized, whole-person care.
- Conduct in-home visits (majority of the time) as well as visits to hospitals, shelters, or clinics to assess patient needs.
- Provide clinical support and health-related education to patients based on their specific conditions and care plans.
- Collaborate with community resources, healthcare providers, and insurance teams to facilitate access to necessary medical and social services.
- Utilize telehealth services when necessary to accommodate patient preferences or scheduling constraints.
- Address social determinants of health, working closely with patients to overcome barriers to care such as transportation, food insecurity, and housing instability.
- Ensure proper documentation and maintain patient records according to compliance standards.
- Work alongside Community Health Workers (CHWs), who focus on patient enrollment, while the Lead Care Manager handles direct patient care and coordination.
Minimum Education and Experience Qualification Requirements:
- Medical Assistant Diploma or CNA Certification Required
- Previous experience in patient coordination, case management, or clinical care preferred.
- Ability to travel throughout Contra Costa County to meet patients in their homes or other settings.
- Strong communication and interpersonal skills to build trust with patients and coordinate care effectively.
- Comfortable using telehealth platforms when required.

Schedule/ Shift: Monday - Friday 8am - 3pm
