Lead Care Manager

What You’ll Do

• Review eligibility and complete enrollment referrals for members.
• Coordinate care across healthcare providers, social services, and community resources.
• Conduct assessments, create care plans, and provide education on treatments, medications, and follow-up needs.
• Maintain accurate documentation in the electronic health record (EHR).
• Regularly meet with members—primarily through home visits—to ensure continuity of care.
• Advocate for members during medical visits when needed.
• Identify and follow up on referrals to ensure member and family needs are met.
• Reassess care plans to ensure they are effective and adjust as needed.
• Respond to member questions, concerns, and requests promptly.
• Recognize and report signs of child/elder abuse as required by law.
• Support members with access to housing, transportation, food, and financial resources.

Qualifications

Must Have: 

• Medical Assistant diploma or Certified Nursing Assistant (CNA) certification
• 2+ years of experience in healthcare or a related field
• Valid driver’s license, clean driving record, and proof of insurance
• Ability to travel locally within Siskiyou County 

 

Nice to Have:

• Bilingual in English/Spanish
• Experience with CalAIM, Enhanced Care Management (ECM), or similar programs

• Comfortable with computers, email, Microsoft Office, and virtual platforms
• Problem-solving and critical-thinking abilities
