Job Summary
Our client is seeking a dedicated Care Coordinator to connect individuals facing serious health challenges with the care they need. You will play a vital role in removing barriers and uplifting vulnerable community members.
Key Responsibilities
- Transitional Care: Support clients as they move from hospital or rehab settings back into the community—ensuring continuity, safety, and support.
- Care Plan Development and Implementation: Conduct initial and ongoing assessments to document client strengths, needs, goals, and resources.
- Connectivity to Care: Schedule and coordinate follow-up with primary care and behavioral health providers.
- Addressing Gaps in Care: Identify and address missed appointments, medications, or unaddressed needs.
- Social Determinants of Health: Link clients to resources such as housing, food security, and benefits support.
- Collaborative Care: Work with providers to build individualized, person-centered care plans.
- Engagement: Provide outreach, engagement, and service planning in the field.
- Documentation: Maintain organized, timely, and accurate documentation as per policy.
Qualifications
- Bachelor’s Degree required in Social Work, Human Services, Psychology, Public Health, or a related field.
- Bilingual preferred.
- One year of related human services experience required, especially with severe mental illness, HIV/AIDS, or other disabilities.
- Ability and willingness to travel regularly, sometimes with clients using reliable and safe transportation.
- Strong communication, organizational, and advocacy skills.

- 1 year of experience in providing direct services to clients diagnosed with severe mental illness, HIV/AIDS, or other disabilities.
