Responsibilities/ Job Duties: 

• Provide field-based case management and psychosocial rehabilitation services to adult clients, reinforcing treatment plans and supporting overall wellness.
• Maintain a caseload of 15-20 clients per week, with regular touchpoints to monitor progress and provide support.
• Attend and contribute to treatment team meetings, collaborating with psychiatrists, clinics, and other team members to coordinate care and determine next steps.
• Transport clients to essential appointments (medical, social services, etc.) and offer in-person support during primary care visits.
• Link clients to community-based resources and services, advocating for their needs and supporting long-term engagement.
• Serve as a liaison between clients and the broader care team, ensuring continuity and consistency in service delivery.
• Actively support team members by providing coverage and collaboration—embracing a “whatever-it-takes” model to meet client needs.

Minimum Education and Experience Qualification Requirements:

• Previous experience in case management, particularly in the mental health field.
• Familiarity with Ventura County resources and systems
• Proven ability to work effectively within a multidisciplinary team, including collaboration with psychiatrists, clinics, and other mental health professionals.
• Valid California Driver’s License 

Schedule/ Shift: Monday - Friday, 8:00 AM - 5:00 PM
