Job Summary:
We are seeking a detail-oriented and patient-focused Referral and Resource Coordinator to join our team. The primary responsibility of this role is to manage and clean up a backlog of referrals while ensuring timely, accurate, and patient-centered coordination of care. This position plays a critical role in closing the loop on open charts—making sure patients complete the next step in their medical journey, whether it be labs, specialty clinic visits, scheduling, or resolving outstanding tasks.
The Referral and Resource Coordinator serves as a centralized liaison between care teams, patients, and specialty clinics in the community.
Key Responsibilities:

Manage and process referral backlog with accuracy and efficiency.

Send referrals to specialty providers and clinics as appropriate.

Work tasks within the EHR, including reading charts and updating records.

Contact clinics and patients by phone to provide updates, answer questions, and coordinate scheduling.

Track referral progress and ensure timely follow-up to maintain continuity of care.

Collaborate with clinical staff, patients, and community providers to ensure smooth transitions of care.

Close out referrals and tasks to support completion of patient care plans.

Maintain compliance with HIPAA, organizational policies, and regulatory requirements.

Build and maintain positive relationships with care teams, patients, and external providers.

Support departmental performance improvement goals and contribute to workflow efficiency.
Qualifications:

High school diploma or equivalent required; associate’s degree or healthcare coursework preferred.

Prior experience in referrals, care coordination, or medical office support strongly preferred.

Familiarity with EHR systems (NextGen experience a plus).

Strong organizational skills and ability to manage multiple priorities.

Excellent communication skills, both verbal and written.
