Job Summary
Our client, recognized as a leader in telehealth services, is seeking a dedicated Registered Nurse (RN) to join their Virtual Care Support team. The RN will play a pivotal role in supporting both patients and clinicians, serving as a valuable resource in guiding patients through their care journeys. Responsibilities include carrying out nursing tasks and clinician-delegated orders to enhance patient care quality.
Key Responsibilities
- Assist patients through the healthcare system as a patient advocate and navigator.
- Participate in patient-centered team meetings and quality improvement initiatives.
- Facilitate patient education on health and disease management.
- Support patient self-management of disease and behavior modification interventions.
- Coordinate continuity of patient care with external organizations and facilities, as well as specialty care referrals.
- Navigate high-risk patient care, including management of patients with multiple co-morbidities.
- Conduct comprehensive, preventive screenings and assist with patient interactions.
- Promote clear communication among care team members and clinicians regarding patient care plans.
- Participate in data collection, health outcomes reporting, clinical audits, and program evaluations related to Virtual Care.
- Evaluate clinical care and develop new clinical tools and procedures.
- Follow standard procedures related to triage, medication refills, and escalation of care.
- Collect and analyze biometric data to support population health and patient care initiatives.
Qualifications
- Bachelor of Science in Nursing with a current RN license in state of residency.
- Compact licensure required, must reside in a compact state with a CA single state licensure.
- Excellent verbal communication skills.
- Proficient computer skills, including experience with Google Suite.
- Experience working in an electronic health record system or with databases.
Experience
- 2-5 years of experience in a clinical setting, preferably in an outpatient clinic with chronic disease management care.
Skills
- Self-disciplined, energetic, passionate, innovative, and flexible.
- Comfortable with ambiguity and working in fast-paced environments.
- Ability to collaborate well within a team and work independently in a remote setting.
- Demonstrates sound judgment, decision-making, and problem-solving skills.
Additional Requirements
- Occasional evening and weekend coverage may be required.
