Job Summary

Our client is seeking an Advanced Practice Clinician (NP or PA) to join the Care Anywhere (CAW) Program. The primary responsibility is to deliver proactive, in-home medical and social support to seniors to prevent avoidable hospitalizations and complications.

Responsibilities / Job Duties

• Provide in-home medical care and social support to seniors.
• Focus on preventing avoidable hospitalizations of the patients.
• Blend fieldwork with administrative duties.

Schedule / Shift

Mon–Fri, ~8:00 a.m.–5:00 p.m.
