Job Summary
Our client is seeking a skilled Health Home Care Manager to provide leadership and direction to the Care Coordination Services Center and Home Health Nursing teams. The role involves ensuring the delivery of efficient, high-quality, and participant-centered care, overseeing clinical and administrative staff, and collaborating with leadership to develop strategies that improve health outcomes.
Responsibilities / Job Duties
- Provides leadership and direction to the Care Coordination Services Center and Home Health Nursing teams, ensuring high-quality care.
- Supervises and supports clinical and administrative staff, including hiring, training, coaching, and performance evaluations.
- Collaborates with senior leadership to develop strategies promoting care continuity and improved health outcomes.
- Oversees the coordination of participant care plans, ensuring seamless transitions between care settings.
- Ensures appropriate management of home health services, including skilled nursing visits and chronic disease management.
- Collaborates with interdisciplinary teams to develop, review, and modify care plans based on participant needs.
- Monitors and manages home health service utilization to optimize efficiency and reduce unnecessary costs.
- Ensures compliance with regulatory requirements and implements quality improvement initiatives.
- Participates in internal and external audits, ensuring adherence to guidelines.
- Serves as the primary liaison to ensure alignment of care delivery processes.
- Utilizes data analytics to monitor performance and implement evidence-based interventions for continuous improvement.
- Prepares and presents reports on key performance metrics.
Minimum Education and Experience Qualification Requirements
Education
- Graduate from an accredited school of professional nursing.
- Bachelor of Science in Nursing (BSN) required; Master’s degree in Nursing, Healthcare Administration, or related field preferred.
- Current California Board of Registered Nursing License required.
- Current CPR-BLS certification required.
Qualifications
- Maintains eligibility for licensure with continuing education units.
- Strong knowledge of PACE regulations and interdisciplinary team collaboration.
- Proven leadership ability in developing care coordination and clinical teams.
- Proficiency in electronic health records, data analysis tools, and Microsoft Office Programs.
- Valid California driver’s license, motor vehicle insurance, and reliable transportation.
Experience
- Minimum of 5 years of nursing experience in acute, skilled nursing facility, or ambulatory care settings.
- At least 3 years in a leadership or management role working with the frail elderly population.
Skills
- Strong verbal and written communication skills.
- Ability to react calmly and effectively in emergency situations.
- Excellent organizational, problem-solving, and decision-making skills.

- Knowledge of common safety hazards and precautions for participants' safety.
