Job Summary
Our client is seeking a dedicated Registered Nurse (RN) to join their Virtual Care Support team. The RN will work alongside other clinical staff to support Virtual Care clinicians and patients, acting as a care guide through the continuum of care, carrying out nursing tasks and orders delegated by clinicians, and serving as a resource for both patients and clinicians.
Key Responsibilities
- Assist patients through the healthcare system as a patient advocate and navigator.
- Participate in patient-centered team meetings and quality improvement initiatives.
- Facilitate health and disease patient education, supporting patient self-management and behavior modification.
- Coordinate continuity of patient care in conjunction with care team members and external healthcare entities.
- Navigate high-risk patient care and manage patients with multiple co-morbidities.
- Conduct comprehensive preventive screenings and support daily patient interactions.
- Promote clear communication among care teams and clinicians.
- Facilitate patient medication management and follow standard procedures and protocols.
- Participate in data collection, health outcomes reporting, clinical audits, and evaluations related to Virtual Care.
- Provide support with clinical guidelines and protocols to medical assistants.
Qualifications
- Bachelor of Science in Nursing with current RN license in state of residency.
- Compact licensure required and residence in a compact state; CA single-state licensure needed.
- Excellent verbal communication and proficient computer skills, including experience with Google Suite.
- Experience with electronic health record systems or databases.
Experience
2-5 years of experience in a clinical setting, ideally in an outpatient clinic with chronic disease management care.

Occasional evening and weekend coverage may be required.
