Job Summary
The Case Manager provides specialized HIV Care Coordination services focused on connecting people living with HIV to essential health, medical, and psychosocial services. This position ensures timely engagement and retention in care, monitors linkage and health outcomes including viral load suppression, and contributes to quality management activities. The Case Manager conducts HIV testing and counseling, actively supports re-engagement efforts, and helps address systemic barriers to care in collaboration with internal and external stakeholders.

Requirements
- Bachelor's Degree in any related field
- Valid driver's license and insurance required
- 2+ years of Case Management experience working with vulnerable populations
- Proficient in Microsoft Suites and EMR/EHR
- Bilingual (English/Spanish) is a plus, but not required
- Experience working with HIV related groups HIGHLY preferred, but not required
Responsibilities / Job Duties
- Review internal referrals to identify clients needing linkage or re-engagement.
- Contact clients via phone or text messages to schedule initial or follow-up appointments.
- Assess client needs and barriers through intake interviews and psychosocial assessments.
- Develop and document individualized linkage or re-engagement plans with measurable goals.
- Monitor client progress and follow up to ensure medical appointments are kept.
- Coordinate transportation or other support services necessary for access to care.
- Make warm handoffs to mental health, substance use, or housing programs as needed.
- Provide culturally competent and trauma-informed support to clients in crisis or transition.
- Maintain regular communication with case managers, health providers, and external partners to monitor client engagement.
- Enter client case notes, service encounters, and outcome data into electronic health records within 24 hours.
- Track HIV viral load labs and update client records with medical outcomes.
- Document linkage timelines, outreach attempts, and successful connections to care.
- Compile client data for program audits and monthly or quarterly reports.
- Provide pre- and post-test counseling in accordance with agency and partnered PS testing.
- Refer clients testing positive to medical care and initiate linkage protocol.
- Maintain confidentiality and comply with all testing documentation requirements.
- Attend weekly case conferences and monthly quality management meetings.
- Contribute client insight and trends to improve service delivery and retention strategies.
- Participate in training, webinars, and professional development opportunities.
- Assist with internal program reviews and data quality checks.
- Attend outreach events, community health fairs, and network meetings to promote services.
- Build and maintain working relationships with healthcare providers, CBOs, and benefits agencies.
- Represent the agency in a professional and mission-aligned manner.
- Identify gaps in referral systems and propose improvements to leadership.
