Job Summary
The Community Outreach Liaison serves as a vital connector between healthcare services and the community. This role focuses on building strong referral relationships, increasing patient census, and promoting brand awareness. Acting as the local market leader for outreach and engagement, the liaison will work with referral partners, patients, and caregivers to ensure seamless communication, education, and enrollment in services.
Responsibilities / Job Duties

Build and maintain strong relationships with referral sources such as hospitals, physician offices, senior living communities, and other healthcare providers.

Identify and pursue new business opportunities to increase patient census.

Represent the organization at community events, health fairs, and networking functions.

Educate patients, families, and caregivers on available services and guide them through the enrollment process.

Facilitate communication between referral sources and internal clinical teams.

Ensure timely follow-up on referrals and maintain accurate documentation of outreach activities.

Monitor market trends and competitor activity to inform strategic planning.
Education
High school diploma or equivalent required; bachelor’s degree in healthcare administration, business, marketing, or a related field preferred.
Qualifications

Minimum 2 years of experience in community outreach, healthcare sales, or business development.

Strong interpersonal and communication skills, with the ability to engage diverse audiences.

Reliable transportation and valid driver’s license required.

Ability to meet or exceed performance targets.

Hospice or home health experience preferred.

Knowledge of Medicare/Medicaid and hospice eligibility criteria preferred.
Experience

Minimum 2 years in a community outreach, healthcare sales, or business development role.

Schedule / Shift
Monday–Friday, normal business hours.
