Job Summary
Our client is seeking a Director of Social Services responsible for coordinating the daily operations of various programs. The role includes providing leadership and guidance to staff and interns, collaborating with departments for seamless client care, and ensuring compliance with policies and procedures. The Director will also be involved in developing comprehensive care plans, conducting outreach to potential clients, and analyzing data to improve program performance.
Responsibilities / Job Duties
- Manage and coordinate daily operations of various programs, ensuring compliance with policies and procedures.
- Conduct comprehensive assessments and develop care plans with a multidisciplinary team.
- Establish and maintain positive relationships with clients to ensure their needs are met.
- Conduct outreach to potential members and coordinate care for clients.
- Monitor and evaluate program effectiveness, provide reports on performance, and recommend strategies for improvement.
- Lead regional growth and expansion of programs with new clients.
- Identify key performance indicators and inspire team members to achieve goals.
- Supervise care managers, staff, and interns and provide support to program managers.
Minimum Education and Experience Qualification Requirements
Education
- Master’s degree in Healthcare, Social Work, or a related field.
Qualifications
- Strong focus on accountability and relationship-building with clients.
- Excellent leadership, supervision, and team management skills.
- Experience in program management and strategic planning.
- Preferred experience in areas such as homelessness, substance use disorders, and mental health issues.
- Experience with Medi-Cal Transformation (CalAIM) and Enhanced Care Management.
