Job Summary
Our client is seeking a dedicated Registered Nurse Case Manager responsible for coordinating patient and family care, including services from various healthcare team members. The role involves ensuring effective care plans, monitoring re-certification processes, and updating care plans in response to changes in patient conditions. The position also requires educating patients and families, performing skilled nursing duties based on interdisciplinary plans of care, and integrating quality improvement results into care plans.
Responsibilities / Job Duties
- Coordinate care and services for patients and families, including team, social worker, chaplain, home health aide, volunteer, and physician involvement.
- Communicate necessary information to the Team Manager.
- Monitor and manage the re-certification process.
- Participate in Interdisciplinary Team Meetings as required.
- Complete accurate and timely documentation.
- Ensure and document appropriate use of medications, durable medical equipment, and supplies.
- Educate patients, families, and other caregivers.
- Provide skilled nursing according to the interdisciplinary plan of care.
- Update care plans regularly to reflect changes in patient condition.
- Utilize contract personnel such as PT, OT, Dietary, Speech, and Pharmacy appropriately.
- Conduct on-site evaluations of Certified Home Health Aides (CHHA) and volunteers.
- Participate in the on-call rotation, including weekends and holidays as needed.
- Incorporate Quality Improvement results into the care plan.
- Contribute to the orientation of new team members and train nursing students when assigned.
- Attend regular in-services and educational conferences.
- Reconcile medication with changes of care level and update electronic health records and Hospice pharmacy.
- Perform additional duties as assigned.
- Maintain accountability for work and portray a positive attitude.
- Avoid gossip and negativity, maintaining constructive communication with others.
- Inform patients and co-workers on how to contact you under normal and unforeseen circumstances.
Schedule / Shift
Participation in on-call rotation, including weekends and holidays, if the situation arises.
