Job Summary
Our client is seeking a Physician to ensure primary care, meet community and all Program of All-inclusive Care for the Elderly (PACE) standards, and directly provide services to participants. The role involves collaborating with the Medical Director, the Certified Registered Nurse Practitioner, and the Interdisciplinary Team (IDT) to assure 24-hour medical coverage through on-call systems.
Key Responsibilities
- Provide oversight, supervision, and collaboration with the CRNP for the provision of medical care to participants.
- Complete comprehensive History and Physical on new participants to determine goals of care and establish end-of-life medical care wishes.
- Conduct annual, semi-annual, and condition-based assessments on established participants.
- Evaluate and treat participants during acute illness consistent with POLST and Advance Directives.
- Participate in the Interdisciplinary Team to manage participants’ medical care and communicate their conditions holistically.
- Oversee the use of medical specialists and inpatient care, ensuring consistent and timely discharge planning.
- Rotate night and weekend calls with other physicians to provide 24-hour/day call coverage for all participants.
- Inform the Medical Director of random and unusual events for discussion.
- Follow policies related to medical record documentation, ensuring timeliness.
- Be involved in the development and implementation of Quality Assessment and Performance activities (QAPI).
- Actively participate in family conferences, team meetings, committees, and work groups as assigned.
Qualifications
- Demonstrated commitment to the mission and core values of assisting the elderly in maintaining dignity and independence.
- Capability to work within Adult Day Health Centers and travel to participant homes, hospitals, and nursing facilities as needed.
- Preparedness to work with individuals that may involve exposure to unpleasant sights and smells, blood-borne pathogens, and other communicable diseases, using Universal Precautions at all times.
