Job Summary

Our client is seeking an Advanced Practice Nurse who will play a pivotal role in providing comprehensive healthcare services to patients throughout their healthcare journey. This position offers the opportunity to make a significant impact on patient outcomes and promote continuity of care. Visits will be completed in-home and/or community-based services such as assisted living facilities, skilled nursing facilities, group homes, or other designated locations.

Key Responsibilities

• Conduct comprehensive in-home assessments for patients, including medical history reviews, physical examinations, and medication management with a minimum of 25 visits per week in assigned territory.
• Conduct thorough assessments of patients' health status, including physical examinations, medical histories, and risk assessments.
• Formulate accurate diagnoses and develop individualized treatment plans based on evidence-based practices and guidelines.
• Manage and monitor chronic health conditions, such as diabetes, hypertension, and cardiovascular diseases, ensuring optimal disease control and patient education.
• Prescribe medications and adjust dosages as necessary, while educating patients on proper medication administration and potential side effects.
• Promote preventive healthcare measures, including immunizations, screenings, and lifestyle modifications, to improve patient health and prevent future illnesses.
• Provide patient education on disease management, healthy lifestyles, and self-care practices to empower patients to take an active role in their health.
• Collaborate closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and seamless transitions for patients.
• Serve as patients' primary care provider.
• Maintain accurate and up-to-date electronic medical records (EMR) to track patient progress, treatment plans, and interventions to be completed within 48 hours of visits.
• Exhibit exceptional bedside manner, fostering trust and rapport with patients and their families through genuine care and empathy.
• Schedule and conduct regular follow-up appointments with patients to assess treatment effectiveness and make necessary adjustments.
• Participate in quality improvement initiatives and adhere to healthcare regulations and standards to maintain the highest level of care quality.
• Participate in after-hours, on-call to conduct longitudinal care and IHA visits as needed.
• Ensure reliable transportation to and from member appointments, adhering to scheduled times and assigned territories.
• Adhere strictly to all company policies, procedures, mission statement, values, Code of Ethical Business Conduct, and Compliance Program.
• Comply with all Federal and applicable State and local laws regarding privacy, confidentiality, and security of health information, and other designated information.
• Other duties as assigned based on business needs.
