We are seeking a detail-oriented RN to assist with Chart Review. This role will focus on retrospective and current chart reviews for medical necessity and documentation compliance. The position is expected to last 6–9 months. 

Key Responsibilities:

• Perform chart reviews to ensure documentation compliance and assess medical necessity.
• Analyze claims for hospital and physician fees.
• Consult with Physician Advisors on complex cases or denials.
• Assist in preventing fraud, waste, and abuse in documentation and claims.
• Comply with HIPAA and DHCS regulatory guidelines.
• Utilize SmartCare EHR system and DCHS criteria.
• Document and save findings in SharePoint and an internal electronic system.
• Attend monthly department meetings.
• May provide departmental coverage if colleagues are out.
• Participate in 1–2 weeks of structured orientation/training.

Case Load:

• Responsible for 5–6 hospitals in California.
• Expected to review 10–15 clients per day.

Qualifications:

• Valid CA RN license Required 
• rior experience in chart review, medical documentation, or claims analysis required.
• Familiarity with hospital and physician billing.
• Ability to navigate EHR systems; experience with SmartCare preferred.
• Knowledge of DCHS criteria and HIPAA compliance.
