Position Summary:
We are seeking a compassionate and experienced Social Worker to join a team dedicated to supporting pediatric patients with diabetes, ranging in age from infancy through young adulthood (0–22 years old). In this role, you will partner closely with an interdisciplinary team of pediatric social workers to provide continuous psychosocial care across both ambulatory and inpatient settings. Your focus will be on case management, psychosocial assessments, care coordination, and family support throughout the patient’s continuum of care.

Key Responsibilities:

Case Management: Provide intensive case management for a high-volume caseload (50+ patients) focused on pediatric diabetes care. Ensure continuity and coordination across services.

Psychosocial Assessments: Conduct thorough assessments to evaluate the psychological, social, and emotional needs of patients and families.

Psychoeducation & Adjustment Support: Offer education and guidance to families adjusting to a chronic illness. Discuss diagnoses and hospitalizations in a developmentally appropriate way with both patients and their families.

Care Coordination & Referrals: Identify and connect families with appropriate community resources, including referrals for material assistance such as temporary housing or financial support.

Interdisciplinary Collaboration: Serve as a vital member of a multidisciplinary healthcare team, contributing to care planning and family meetings.

Crisis Intervention: Provide support in times of crisis, including grief, bereavement, and critical changes in medical condition.

Family Advocacy: Act as an advocate for children and families to ensure their needs and rights are represented and met within the healthcare system.

Care Planning: Develop and implement individualized pre- and post-hospitalization care plans.

Documentation: Maintain detailed, accurate, and timely case documentation in accordance with institutional standards.

Promoting Safety: Support efforts to maintain a safe and supportive environment for patients and families.
Qualifications (Must-Have):

Master’s degree in Social Work (MSW) from an accredited school of social work

Licensed by the Maryland Board of Social Work Examiners at the LMSW level or higher

Experience in a medical or clinical setting
