Job Summary

Our client is seeking a dedicated Infusion Specialist who will work independently at a designated site. The candidate will have the opportunity to initiate, develop, and implement nursing plans, administer treatments, and educate patients and caregivers. While this position starts as a contract role, there may be potential for conversion based on the clinic's needs.

Key Responsibilities

• Develops and implements nursing plans and evaluates patient progress towards goals.
• Organizes and provides direct patient care, performs treatments, and administers medications.
• Assesses and re-evaluates patient needs, clarifies physician orders, and communicates changes in patient status.
• Provides patient-centered care education to achieve patient and family independence with prescribed therapies.
• Coordinates discharge planning and prepares patient instructions and summaries.
• Ensures compliance with agency operations, policies, professional standards, and regulations.
• Utilizes technology to document and communicate patient care efficiently.
• Participates in on-call responsibilities and facility process improvement programs.

Qualifications

• Demonstrates ability to work independently at a remote site.
• Strong ethical and professional conduct in line with legal and regulatory guidelines.
• Proficient in using technology for patient care documentation and communication.
• Ability to provide oversight and coordinate paraprofessionals in home settings.

• Excellent communication skills for interacting with clinical teams, medical providers, patients, and families.
• Technical proficiency in completing documentation and managing patient data in real-time.
