Job Summary

Our client is seeking a Nurse Case Manager to join their Nurse Family Partnership (NFP) program, which is a nationally recognized, evidence-based nurse home-visiting program. The primary responsibilities include assessing the medical and social needs of pregnant women and infants, following a prescribed process/plan of care, and monitoring outcomes in accordance with NFP and client policies and procedures.

Key Responsibilities

• Provide home visits to pregnant/parenting women and their families eligible for the NFP Program.
• Provide education and case management services from pregnancy through the child’s second birthday.
• Adhere to the nursing process and the NFP model of home visitation.
• Maintain a caseload of at least 25 clients.

Clinical Duties

• Develop therapeutic relationships using reflection and motivational interviewing.
• Perform home visiting in accordance with the NFP model and guidelines.
• Assess needs related to NFP domains and assist families in goal setting.
• Provide education, support, and referrals to help families attain their goals.
• Consult and collaborate with other professionals and follow up on service delivery.
• Complete all required documentation for Medicaid billing.

Qualifications

• BSN required or associates if currently enrolled in a BSN program.
• Current New York State RN license (in good standing) is required.
• Current CPR certification.
• Bilingual English/Spanish preferred.
• Home visiting experience preferred.
• Excellent written and verbal communication skills.
• Basic computer skills.

Additional Requirements

• Valid driver’s license and insured automobile may be required.
• May require working a flexible schedule including some evenings and weekends.
