Job Summary
Our client is seeking a Case Manager specializing in Linkage Coordination (LTC) to provide HIV Care Coordination services. The role involves connecting individuals living with HIV to essential health, medical, and psychosocial services. The position focuses on ensuring timely engagement and retention in care, monitoring linkage and health outcomes including viral load suppression, and contributing to quality management activities. The LTC Case Manager will conduct HIV testing and counseling, support re-engagement efforts, and address systemic barriers to care in collaboration with internal and external stakeholders.
Responsibilities / Job Duties
- Review internal referrals to identify clients needing linkage or re-engagement.
- Contact clients via phone or text to schedule initial or follow-up appointments.
- Perform intake interviews and psychosocial assessments to evaluate client needs and barriers.
- Develop and document individualized linkage or re-engagement plans with measurable goals.
- Monitor client progress and ensure medical appointments are kept.
- Coordinate transportation or other support services necessary for access to care.
- Provide culturally competent and trauma-informed support to clients in crisis or transition.
- Maintain communication with case managers, health providers, and external partners.
- Enter client case notes, service encounters, and outcome data into electronic health records within 24 hours.
- Track HIV viral load labs and update client records with medical outcomes.
- Document linkage timelines, outreach attempts, and successful connections to care.
- Compile client data for program audits and reports.
- Conduct pre- and post-test counseling and refer clients testing positive for medical care.
- Participate in multidisciplinary team meetings and quality management activities.
- Engage in community outreach events and build relationships with healthcare providers and agencies.
