1. Responsibilities/ Job Duties: 
2. Assist provider with rooming patients and general clinical support
3. Perform immunizations, EKGs, and obtain vital signs
4. Coordinate and participate in monthly Diabetes Clinic (2nd Friday of each month)
5. Conduct patient outreach and ensure preparedness for diabetes workshops
6. Help facilitate and run the monthly workshop
7. Minimum Education and Experience Qualification Requirements: 
8. Active CA LVN License
9. 2+ years of LVN or relevant clinical experience preferred
10. Schedule/ Shift: Monday – Friday, 8:30 AM – 5:30 PM
