Job Summary

Our client is seeking a dedicated Clinician to provide comprehensive treatment and assessment services. The ideal candidate will facilitate group treatments, develop and manage treatment plans, and coordinate care with healthcare professionals and family members. They will ensure accurate documentation and compliance with all program policies and procedures.

Responsibilities / Job Duties

• Effectively facilitate group treatment modalities within the scope of practice, using approved curriculums.
• Conduct initial assessments of new patients and develop initial treatment plans.
• Develop and update master treatment plans using data from assessments and team input.
• Enter progress notes accurately and in a timely manner.
• Document incidents or unusual occurrences promptly.
• Ensure completeness and accuracy of medical records for assigned patients.
• Complete discharge summaries as per program policies and procedures.
• Participate in public relations and educational activities as assigned.
• Assess the needs of adult patients and facilitate movement to the next treatment stage.
• Develop therapeutic relationships and maintain regular contact with others involved in patient’s care.
• Monitor patient attendance and follow up on attendance issues.
• Assist in counseling patients about funding issues.
• Coordinate with attending physicians regarding treatment and discharge plans.
• Follow mandatory reporting laws for abuse and domestic violence.
• Manage patient crises situations effectively.
• Execute additional duties and responsibilities as required.

• Excellent communication skills for liaising with healthcare professionals and families.
