Job Summary

Our client is seeking a Licensed Social Worker to join a collaborative team of behavioral and physical health clinicians. The primary goal of this role is to deliver exceptional patient care through proven whole-patient care strategies.

Key Responsibilities

• Be a critical part of a collaborative team of behavioral and physical health clinicians committed to delivering exceptional patient care.
• Partner with primary care providers to deliver proven whole-patient care.
• Provide brief, structured intervention techniques such as Behavioral Activation, Motivational Interviewing, Solution Focused, and evidence-based psychosocial treatments like Cognitive-Behavioral Therapy and Interpersonal Therapy.
• Leverage real-time data to drive positive patient outcomes, immediately understanding your impact on the patients you serve.
• Positively contribute to an ever-expanding clinical team all committed to a new, innovative way to serve patients.

Qualifications

• Licensed as a Social Worker.
• Experience with evidence-based psychosocial treatments.
