Job Summary
Our client is seeking a Bilingual Coordinated Family Support Instructor (Case Manager) to join a family-oriented company that provides services to intellectually and developmentally challenged adults in their homes and community. The primary goal is to support individuals in participating in daily routines and recreational activities, ensuring a safe and healthy living environment.
**Language Requirement: Seeking candidates who speak English and one of the following languages: Armenian, Cantonese, Mandarin, Vietnamese, or Tagalog
Shift: Mon-Fri (8:30am-5pm)
Location: Burbank, CA
Driving: Yes - home visits, mileage reimbursement provided
Responsibilities / Job Duties
- Work with the individual, their family, and members of their "circle of support" to ensure safety and health by focusing on their needs.
- Familiarize with community resources to support the individual's needs and provide training and habilitation to fulfill their desired outcomes.
- Document and complete all reporting accurately and engage in proactive communication with key stakeholders regarding the individual's needs and progress.
- Participate in all required training, meetings, and coaching sessions to develop knowledge, skills, and abilities.
- Take ownership of behavior and coach others in demonstrating C.A.R.E. values: compassion, adaptability, respect, and empowerment.
Skills/Requirements
- Must have a valid Driver's License and Auto Insurance.
- Experience working with developmentally disabled clients or in a case management position highly preferred but not required.
