Job Summary
Our client is seeking a skilled Clinician to join their team. The primary responsibilities of this role include facilitating group treatment modalities, conducting initial assessments, developing and updating treatment plans, and maintaining accurate documentation. The Clinician will also be responsible for coordinating patient care with other professionals and managing patient crises situations.
Responsibilities / Job Duties
- Effectively facilitate group treatment modalities using approved curriculums and standard group facilitation techniques.
- Conduct initial assessments of new patients and develop initial treatment plans.
- Develop master treatment plans in a timely manner, using data from assessments and input from the treatment team.
- Review and update treatment plans for assigned patients as prescribed by program policies and procedures.
- Utilize treatment planning meetings to review problems, progress, and discharge plans for assigned patients.
- Accurately enter progress notes within established timeframes and ensure they are legible and address treatment goals.
- Document incidents or unusual occurrences promptly.
- Monitor medical records to ensure completeness and accuracy, notifying the Program Director of any issues.
- Complete discharge summaries for assigned patients in a timely manner.
- Participate in public relations and educational activities as assigned.
- Demonstrate competency with the age-specific needs of adult patients and effectively assess their needs.
- Develop therapeutic relationships with patients and schedule regular 1:1 interactions to review goals and progress.
- Maintain regular documented contact with patients' care team and act as a liaison for necessary information.
- Monitor patient attendance and follow up on attendance issues promptly.
- Gather information and implement behavioral contracts as needed.
- Assist in counseling patients about funding issues as requested.
- Coordinate with attending physicians regarding treatment and discharge plans.
- Follow mandatory reporting laws for abuse and domestic violence in a timely manner.
- Adhere to all billing procedures and conduct insurance reviews effectively.
- Manage patient crisis situations effectively.
- Perform additional duties and responsibilities as required.
