If so, we invite you to join our expanding Local Health Department Assessment Team as a Full-Time Community Personal Assistance Services (CPAS) Nurse Monitor in Baltimore City, MD!
This is an exciting hybrid opportunity to transition from high-stress clinical environments and make a tangible impact on the lives of individuals at risk of institutionalization. As a CPAS Nurse Monitor, you will play a vital role in ensuring participants receive the support and services they need to maintain their independence in the comfort of their own homes.
Pay: $45-50/hour
Case Load: 18-21 patients/week
Schedule/Shift:
40 Hours Weekly, M-F. Nurses are assigned a scheduler that will make appointments for them according to their geographical area and coverage assignment. Nurses must do all appointments!
ESSENTIAL DUTIES:
As a crucial guide and advocate, you will shape how participants interact with support systems by:
- Conducting comprehensive assessments: Analyzing medical and psychosocial information, performing in-home visits, and evaluating participant needs and the quality of services they receive.
- Developing personalized care plans: Collaborating with participants to access home and community-based services that empower them to maintain independence in the least restrictive environment.
- Coordinating and connecting: Referring participants to essential resources, consulting with healthcare professionals, and coordinating services to ensure a seamless and effective support system.
- Championing participant autonomy: Advocating for participant rights, promoting self-determination, and ensuring fair access to vital services.
- Documenting for impact: Maintaining timely and accurate records to track progress, inform service decisions, and contribute to the ongoing improvement of the CPAS program.

Overview of the Nurse Monitoring Program:
Our Nurse Monitoring Program provides essential services to individuals receiving personal care assistance and other home and community-based supports funded by the Maryland Department of Health. Our dedicated Registered Nurse Monitors conduct comprehensive in-home assessments that encompass medical, nursing, functional, psychosocial, and environmental factors. These assessments are crucial in determining the appropriate level of care and the frequency of ongoing nurse monitoring visits. You will work closely with participants, their Supports Planners, family members, and other clinicians to effectively address their unique needs.

3-5 Must-Have Skills/Qualifications:
- EDUCATION AND LICENSING REQUIREMENTS: Holds a valid State of Maryland RN license. Possession of a valid license to practice as a professional registered nurse, issued by the Maryland State Board of Examiners of Nurses, is required at the time of appointment.
- Has a minimum of 2 years of experience in medical-surgical, geriatric, and/or pediatric case management.
- Must be able to pass a thorough background investigation.
- Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment. All licenses and certifications must be maintained at current levels throughout employment.
