Job Title: ECM Director (RN/LVN)

Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Start Date: Remote to start; in-office 5 days/week once LA office opens (expected May)

About the Role

Medix is seeking a highly experienced and licensed ECM Director to oversee and lead Enhanced Care Management (ECM) operations in the Los Angeles region. This key leadership position will be responsible for managing relationships with local health plan partners, auditing program processes, supporting staff performance, and ensuring compliance with CalAIM requirements. The ECM Director will bring subject matter expertise in ECM and play a pivotal role in improving care coordination and population health outcomes for vulnerable populations.

Key Responsibilities

• Serve as subject matter expert on ECM and CalAIM program requirements, updates, and practices.

• Lead ECM planning and operations in collaboration with health plan partners and organizational leadership.

• Ensure compliance with regulatory and contractual guidelines by regularly auditing care plans and internal workflows.

• Develop, monitor, and report on key performance indicators (KPIs) for ECM program effectiveness.

• Identify areas for operational process improvement and drive implementation of best practices.

• Act as liaison between health plans, external partners, and internal teams to promote collaboration and service integration.

• Assist in stakeholder engagement including scheduling, coordination, meeting facilitation, and follow-up documentation.

• Support staff by developing and implementing culturally responsive training and performance improvement strategies.

• Regularly report to leadership on ECM performance, outcomes, and operational challenges.

• Participate in strategic planning and contribute to the alignment of ECM efforts with organizational goals.

• Perform other related duties as assigned.

Minimum Requirements

• Active California RN or LVN License (Required)

• Strong knowledge of ECM and CalAIM program requirements

• Experience in disease case management, public health, or working with agencies like LAHSA

• Proven experience with care plan reviews, health plan compliance, and interagency collaboration

• Healthcare background with a focus on operational process improvement

• Strong written and verbal communication and presentation skills

• Proficient in Microsoft Office (Word, Excel, Outlook); digital literacy required

Preferred Qualifications

• Experience in a supervisory or leadership role within a healthcare or ECM setting

• Familiarity with health plans and community resources in the LA region

• Bilingual in Spanish (preferred, but not required)
