Job Summary

Our client is seeking a dedicated Registered Nurse Case Manager to plan, organize, and deliver nursing services in collaboration with clinic providers. The primary goal of this position is to improve the health outcomes of patients with chronic disease conditions and/or complex needs through effective coordination of care and resource management.

Key Responsibilities

• Plan, organize, and provide nursing services to clinic patients.
• Collaborate with providers and other healthcare team members to improve patient health.
• Educate patients and families on managing health care needs.
• Facilitate communication and coordinate services to address barriers.
• Promote optimal allocation of resources while balancing clinical quality and cost management.
