Job Summary
Under the direct supervision of the CEO, the Community Connector will be responsible for the oversight and development of the service teams. This includes collaboration with a number of local entities to provide integrated services and care management for clientele. The role assures effective day-to-day operations within various programs and supervises staff in their region. The Community Connector will also provide training and modeling for staff, conduct weekly caseload audits for program clientele, and ensure compliance with relevant laws and policies.
Responsibilities / Job Duties
- Oversee and coordinate resource allocation, record keeping, and reporting functions, ensuring compliance with stated objectives.
- Evaluate program operations for effectiveness and compliance, implementing improvements as necessary.
- Develop and participate in collaborative activities to maximize resources and client responsiveness.
- Provide supervision for all staff, identify training needs, and ensure the development of necessary programs.
- Develop and manage systems to maximize the program's effectiveness and provide support to staff and clients.
- Collaborate with other units and partners to achieve program goals within agreed upon timeframes.
- Participate in budget preparation and administration, monitoring and controlling expenditures.
- Perform screenings, complete individualized care plans, and coordinate care with case management staff.
- Participate in meetings to address client needs and assist in service integration.
- Assist in preparing grants to expand CalAIM services.
- Participate in relevant meetings and trainings.
- Ensure cultural sensitivity and adherence to client confidentiality in all interactions.
- Perform other duties as assigned.
Minimum Education and Experience Qualification Requirements
Education
- Bachelor's Degree in Human Services field (Social Work, Psychology, Sociology, Public Health, etc) required; Master's Degree preferred.
Qualifications
- Five years of experience in human services, behavioral health, or related programs, including two years of supervisory responsibility.
- Experience with individuals with mental illness and substance abuse, knowledge of co-occurring disorders and evidence-based treatments.
- Proficient in program development and evaluation, familiar with state and federal mental health service regulations.
- Knowledgeable in social service case management and outcome measurement.
