This role provides essential support in facilitating communication and collaboration among the healthcare team, patients, and their families. The primary responsibility is to identify and address the physical, and emotional needs of the patient. The position focuses on promoting optimal health, well-being, and safety, ensuring care aligns with established standards, guidelines, and the delegation of medical services as outlined by the State Medical Practice Act.
Key duties include performing a range of clinical tasks under the direction of an authorized healthcare provider, in line with the Advent Health Physician Group Scope of Practice. The role may also involve assisting with front office operations, scheduling patient appointments, and coordinating necessary tests.
Accurate documentation in patient charts and maintaining medical records is essential. Participation in in-service presentations and seminars is encouraged to support ongoing personal and professional development. Demonstrating accountability for relationship-based care, while upholding the organization's mission and core values, is expected at all times.
Responsibilities/ Job Duties:
Assess the physical, psychosocial, cultural, and spiritual needs of patients through observation, interviews, and review of medical records and clinical data. Evaluate patient responses and communicate findings to the responsible healthcare provider to ensure the best possible outcomes.
Contribute to the development and execution of individualized patient care plans, collaborating with the interdisciplinary team. Identify potential barriers to patient engagement and self-management of health conditions, aiming to enhance care planning and coordination.
Demonstrate proficiency in various therapeutic and diagnostic interventions, including but not limited to vital signs monitoring, minor treatments and procedures, preventive services, pre-visit planning, post-visit follow-ups, and medication refills according to existing treatment protocols. Ensure efficient patient flow within the clinic.
Actively utilize patient registries to assess and address ongoing healthcare needs, integrating this tool into practice for optimal patient management.
Advocate for and collaborate with the healthcare team to safeguard the health, safety, and rights of each patient.
Minimum Education and Experience Requirements:
- High School Degree
- Medical Assistant Degree
- BLS Certification
Schedule/ Shift:
- M-F (8:00am-5:00pm)
- Day shift
