Job Summary

The Correction Case Manager/Austin CBC Supportive Case Manager will provide a range of client-centered, confidential services that link clients with health care, clinical psychosocial, and supportive services for correction and non-corrections clients living with HIV/AIDS. This role involves working closely with health centers and medical teams to stabilize clients medically and facilitate their linkage to primary medical services. Responsibilities also include treatment adherence counseling for HIV/AIDS treatments and ensuring clients are connected to core services such as dental, mental health, and substance abuse treatment.

Key Responsibilities

• Complete intake and assessment of client needs, including enrollments and consents.
• Develop and manage comprehensive service plans in collaboration with clients.
• Maintain regular contact with clients to ensure medical and medication compliance.
• Make appropriate referrals and coordinate services as needed.
• Facilitate outreach and engage clients in re-entry support groups.
• Document all client interactions and maintain records in electronic databases.
• Enhance professional development and participate in mandatory training.

Qualifications

• Associate degree required.
• Licensed or License Eligible in the State of Illinois, LSW or LCSW preferred.

Experience

• Experience working in a hospital or outpatient setting preferred.
• Experience with HIV programs and correctional populations preferred.

Skills

• Proficiency with Microsoft Office, Access, and similar databases required.
• Strong oral and written communication skills, problem-solving, and conflict-resolution skills.
