General Overview:
We are seeking a dedicated and compassionate Bilingual Advanced Practice Provider (NP/PA) to deliver high quality medical care to Spanish speaking individuals . The provider will assess, diagnose, plan, intervene and evaluate adult patients of all ages, regarding their medical and mental health conditions. .
Essential Functions:
- Completes and documents an accurate history and physical examination of patients.
- Conducts comprehensive or episodic health assessments and develops patient care plans.
- Provides primary prevention strategies and management of chronic health problems.
- Maintain a current problem list, medication list and immunization record on all patient records.
- Orders and interprets tests, treatments, and therapies within the scope of practice.
- Prescribes medications, diet, supplies, prosthetic devices, and other home-health aides or
- medication according to clinical privileges.
- Provides clear, concise, and accurate information regarding patient care in the medical record.
- Provides education to patients, families and significant others using established education materials.
- Participate in quality assurance and peer review activities, as well as medical and general staff meetings.
- Must recognize and evaluate situations which call for the immediate attention of the attending and/or supervising physician and institute treatment procedures essential for the life of the patient by following the direction of the attending and/or supervising physician.
- Advocates for patients, families, co-workers, the community and the healthcare profession.
- Integrates evidence-based knowledge and clinical expertise in decision-making.
- Complete the following documents, as requested by patients: disability forms, INS letters, letters of disability, DMV handicap forms, IHSS forms, etc...
- Maintain current knowledge of state legal regulations for nurse practitioner practice. Keep abreast of development in nursing. Maintain DEA, licensing and CME/CEUs. Keep abreast of regulatory processes and payer systems.
Job Requirements
- Minimum of 2 years of experience as a NP/PA in community health, FQHC, or family medicine preferably with adult populations
- Bilinguall in Spanish is required.
- Proficient with technology and Electronic Health Record (EHR) systems.
- Education: A master's or doctoral degree from an accredited program in Nurse Practitioner or Physician Assistant.
- Licensure:
- Unrestricted California license and valid DEA number.
- Certification as a Nurse Practitioner or Physician Assistant through an accredited program
- BLS Certification
- Furnishing License
