Telehealth Care Coordinator

This job will require a CNA, Case Manager, or an individual with a clinical background to hold up the screen during the visits. This will be non DPC, and they will be required to drive to different facilities within the New Jersey area. This will be a month to a month and a half assignment

Role & Responsibilities:
- Arrive in a timely manner.
- Reliable and communicates well.
- Facilitate virtual visits.
- Support remote NP/PA.
- Must bring your own personal device/tablet/laptop and charger to visit.
- Wear appropriate professional attire (must be scrubs)
Equipment:
- iPad, tablet, or cell phone.
- No cracked screen.
- Fully charged device.
Day Before Initial Patient Visit (IPV):
Tapestry Health scheduler will confirm with you Facility Name, Address and send Doxy
me link along with CNA Script via email and/or text.

Workflow:
8:45 am- Arrive at Facility and Clock in
8:45-9:00 am- Meet with Director of Nursing (DON)/Admin/Greeter
- Obtain patient list.
- DON/Admin/Greeter will provide Guest Password for the WIFI.
9:00 am-5:00 pm- Perform IPV visits with NP.
- Start at the top of the patient list.
- Identify patient.
- Use CNA introduction script.
- Connect with NP via Doxy me link sent.

Lunch Break -Facility Dependent (30 minutes)
5:00 pm-Return patient list to DON/Admin/Greeter & Clock out

*Mileage will be reimbursed

*There are positions in North Jersey (Passaic and Bergen County) as well as Monmouth and Ocean County
