Job description

Outpatient Long-Term Care Pharmacist opening!

Location: Louisville, KY

Shift: 2nd shift Monday-Friday - 11a-7:30p but can be flexible on start time 10a-11a

Pay: Typical pay ranges from $50-60/h and varies based on years of experience and education

Requirements: Pharm D and state license with no incumbents

Open to experienced in LTC setting or inpatient setting! Also open to retail Pharmacists with at least 1 year of experience

Position Overview: Supports the Pharmacy team by preparing medications, interpreting physician orders, and ensuring the proper weighing, measuring, and mixing of ingredients. Identifies and resolves therapeutic incompatibilities.

Key Responsibilities:

  1. Medication Preparation and Dispensing:
  • Reviews and interprets physician orders for medication preparation.
  • Weighs, measures, and mixes ingredients as needed.
  • Detects and addresses any therapeutic incompatibilities.
  • Plans, implements, and maintains procedures for mixing, packaging, and labeling pharmaceuticals according to policy and legal requirements to ensure quality, security, and proper disposal.
  1. Quality Control:
  • Assesses the identity, strength, and purity of medications.
  • Monitors drug therapies and advises on necessary interventions.
  • Manages the workflow, including pharmacist verification, labeling/packaging, quality assurance with barcode verification, drug images, and delivery with barcode tote checking.
  1. Record Maintenance:
  • Assists in maintaining pharmacy records, including pharmacy files, patient profiles, charge system files, inventories, and controlled drugs/narcotics records.
  1. Regulatory Compliance:
  • Ensures compliance with state and federal drug laws as regulated by the State Board of Pharmacy, the Drug Enforcement Administration, and the Food and Drug Administration.
  • Monitors unit inspections, maintains records for controlled substances, and follows guidelines for drug disposal.
  1. Safety and Security:
  • Protects patients and staff by adhering to infection-control protocols.
  • Conducts unit inspections and removes outdated and damaged drugs from the pharmacy inventory.
  1. Technical Support:
  • Utilizes computer systems to screen for drug interactions, therapeutic duplication, drug-disease contradictions, drug allergies, geriatric precautions, drug-food precautions, and adult min-max dose checking.
  1. Customer Service and Education:
  • Provides pharmacological information and counseling to healthcare professionals and patients.
  • Develops staff knowledge by participating in clinical programs and training pharmacy staff, interns, residents, and healthcare professionals.
  1. Operational Support:
  • Organizes and directs technicians' workflow, verifying their preparation and labeling of pharmaceuticals.
  • Completes pharmacy operational requirements efficiently.
  1. General Duties:
  • Meets expectations of excellence for the position.
  • Supports departmental goals and overall company objectives.
  • Performs all duties with a friendly, positive, and team-oriented approach.
  • Minimal travel, including overnight stays as necessary.
  • Complies with all laws, regulations, and standards of ethical conduct.
