Job description

The job posting is for a Collaborative Care Social Worker, located in the Bronx, NY. It's a full-time, in-person position requiring the candidate to work five days a week at their facility. Company focuses on providing evidence-based mental health services, particularly for patients with anxiety and depression, in a team-based, holistic care environment.

Key Responsibilities:

  • Conduct psychosocial assessments and create personalized care plans for patients.
  • Use evidence-based interventions (like CBT, Behavioral Activation, and mindfulness) to address anxiety and depression.
  • Collaborate with a multidisciplinary team, including primary care providers and psychiatrists, to ensure continuous, effective care.
  • Provide patient education and emotional support, empowering individuals to manage their mental health.
  • Monitor patient progress, adjusting interventions as needed, and ensure proper documentation.
  • Advocate for patients, ensuring access to appropriate resources and reducing mental health stigma.

Qualifications:

  • A Master's degree in Social Work (LMSW) or related field, or Licensed Clinical Social Worker (LCSW) in NY.
  • At least 2 years of experience working with anxiety and depression populations, ideally in a collaborative or integrative care setting.
  • Proficient in evidence-based practices (CBT, mindfulness), communication, and electronic health records (EHR) systems.
