Job description

Medix has partnered with a leading hospital group located in the Chicago area looking for a Licensed Social Worker or Licensed Social Worker to join their tenured team. This individual will report into the Director of the Case Management Department.

Schedule/Shift: Monday-Friday, 8am-4:30pm - Rotating Weekends & Holidays (fully onsite)

Pay: $29-38/hr - dependent on experience


  • Background and understanding of managed care have worked in an acute care setting.
  • This individual will assess health status, level of functional independence, medical needs, and rehabilitation potential for all patients.
  • This individual will also develop supportive environment plans for the patient and family, recognizing individual needs of both.
  • They will also provide information necessary for the patient and family to understand specific illness and its treatment demands of both the patient and family.
  • This will include awareness of the patient and family's resources and providing essential external resources when necessary.
  • This individual will serve as the key patient/caregiver liaison.
  • Specifically looking for someone who has a background in discharge planning in a hospital setting.

Must Have Skills/Qualifications

  • LCSW or LSW

Competencies and Qualifications:

  • Graduation from an accredited school of Social Work with Master's degree
  • Current social work license in State of Illinois preferred or obtained within first year of hire
  • CPR certification'
  • Covid Vaccination: Vaccine required


In order to be eligible for health benefits, you must be employed for 30 days and must average 30 hours per week over your first four weeks on assignment. If you become eligible and take action to enroll, you will be covered no earlier than 60 days into your assignment, depending on plan selection
