Job Description

Serving the diverse and underserved populations of the Rockaways and Five Towns in Queens and Nassau County, NY. We are seeking a bilingual (Spanish-English) LCSW or LMSW to join our Behavioral Health Outpatient team. This position offers the opportunity to make a meaningful impact on patients by providing critical mental health services in a hospital setting.

Responsibilities/ Job Duties:

• Conduct psychosocial evaluations and develop treatment plans
• Provide individual and group psychotherapy in accordance with treatment plans
• Engage families and care partners in patient-centered treatment plans
• Develop aftercare plans and connect patients to community resources
• Ensure compliance with Suicide Risk Assessments and Telehealth Services Policies
• Maintain thorough and timely clinical documentation
• Participate in departmental meetings and interdisciplinary collaboration

Minimum Education and Experience Qualification Requirements:

• Master of Social Work (MSW)
• Valid NYS LCSW (preferred) or LMSW
• Bilingual (Spanish-English) - Required
• 2+ years of experience in a Behavioral Health Outpatient setting
