Responsibilities
- Collaborate with ACT Team members to deliver evidence-based mental health services to individuals and their families enrolled in the program.
- Conduct home and institutional visits to engage clients and family members.
- Utilize mental health screening tools to assess client and family needs.
- Develop, implement, monitor, and document individualized treatment plans that include family-focused strategies.
- Provide families with appropriate referrals and educational resources.
- Maintain accurate and timely documentation using electronic systems (EHR, Google Docs, Microsoft Excel, etc.).
- Build and maintain collaborative relationships with external agencies, services, and programs.
- Actively participate in clinical supervision, team meetings, and interdisciplinary conferences.
- Contribute to staff development, program planning, and agency events as needed.
- Represent the organization at external meetings and community events.
- Ensure that all services meet regulatory standards and agency policies.
- Rotate on-call duties as part of the team to provide 24/7 crisis response.
- Perform additional duties as assigned.
2 Social Work Opportunities:
(1) Family Specialist Qualifications
- Master's degree in Social Work, Mental Health Counseling, or a related field required.
- New York State professional license (LCSW, LMSW, LMHC, or equivalent) preferred; candidates actively pursuing licensure are encouraged to apply.
- Experience working with families and children is essential.
- Strong interpersonal, communication, and organizational skills.
- Ability to work independently and collaboratively in a high-performance team environment.
- Sensitivity to cultural, racial, and individual differences and values.
- Basic computer skills; familiarity with electronic client record systems preferred.
(2) Substance Use Specialist Qualifications
- Master's degree in Social Work, Mental Health Counseling, or a related field required.
- CASAC-T certification or significant experience working with substance use issues is required.
- New York State professional license (LCSW, LMSW, LMHC, or equivalent) preferred.
- Two years of experience in a mental health or substance use treatment setting preferred.
- Excellent communication, problem-solving, and organizational skills.
- Ability to work in a culturally sensitive and inclusive manner.
- Basic computer proficiency; familiarity with electronic client record systems preferred.
