We are hiring a Social Work Case Manager (LGSW - DC License). The Care Manager is responsible for providing comprehensive care coordination services to members who have moderate to complex medical and psycho-social needs enrolled in the health plan.

The Case Manager routinely communicates with members on their assigned caseload to assess their needs; develop, monitor and evaluate treatment plan progress; and assist with coordinating care and referring to appropriate services for members who have moderate to complex care needs.

The candidate needs to hold an active Social Work License (LGSW) in DC to be considered for the role.

SPECIFICS:

• Requirements: Active Licensed Graduate Social Work(LGSW) (DC License is required)
• Hours: Monday - Friday 8:30 AM - 5:00 PM (Full time)
• Hybrid Based: A mix of remote and field work; field work as needed based on acuity of patients
• Location: Washington District Of Columbia 20005 United States
• Pay: $68,000 - $75,000 (negotiable - depending on experience)

DUTIES:

• Develop and implement individualized care plans for children and youth with special healthcare needs.
• Conduct thorough assessments of the physical, emotional, and social needs of each member.
• Provide emotional support and guidance to help them navigate complex healthcare systems, community resources, and entitlement programs.
• Generate reports to inform healthcare professionals and insurance providers about the client's progress and needs.
• Provide education and training to clients and their families on topics related to healthcare management, disease prevention, self-advocacy, and available resources.
