Role Overview
The Shared Housing Navigator role requires a strong working knowledge of available resources and is dedicated to linking participants with necessary programs to achieve stable, permanent housing. This role involves facilitating "active" linkages across various stages of housing establishment, following the Housing First model. Navigators act as advocates, referral agents, and facilitators, working with property owners, government agencies, homeless services providers, and community stakeholders to support participant success.
Essential Duties and Responsibilities
- Participant Intake and Assessment
- Coordinate participant intake and needs assessments.
- Guide participants through the Coordinated Entry System (CES) process from initial intake to securing permanent housing.
- Ensure a smooth transition for participants, positively impacting their housing journey.
- Permanent Housing Identification
- Identify suitable housing options based on initial and ongoing assessments of participant needs, preferences, and barriers.
- Analyze and understand participant factors to secure the best housing solutions.
- Comprehensive Support and Resource Access
- Provide information, referrals, linkages, and advocacy for participants in areas such as health, mental health, substance abuse treatment, benefits, employment, and transportation.
- Ensure participants have access to resources necessary for housing stability.
- Assistance with Documentation
- Assist participants in obtaining necessary documents like identification cards, birth certificates, social security income, and disability income.
- Housing Search and Financial Assistance
- Aid participants in searching for affordable housing, completing rental applications, and applying for one-time funding sources for move-in costs if eligible.
- Collaboration with Service Providers and Stakeholders
- Develop strong working relationships with a range of service providers, including property owners, management companies, and employers, to support the shared housing mission.
- Participant Follow-Up and Goal Support
- Maintain regular monthly contact with participants (adjusted per need), focusing on goal evaluation and support to promote successful housing outcomes.
- Shared Housing Portal Assistance
- Support clients and referral partners in navigating the Shared Housing online portal, offering technical assistance as needed.
- Outreach and Community Engagement
- Conduct outreach to potential clients, service providers, landlords, and community members through phone calls, emails, newsletters, presentations, trainings, and resource fairs.
- Case Documentation and Confidentiality
- Complete case notes and documentation in a timely and professional manner, ensuring participant privacy and confidentiality per HIPAA regulations.
- Participant File Maintenance
- Keep participant files and records organized, safeguarding privacy and ensuring compliance with confidentiality standards.
- Team Meetings and Professional Development
- Participate in staff meetings, case conferencing, and training as directed by the supervisor.
- Application of Best Practices
- Apply best practice techniques, including Trauma-Informed Care (TIC), Motivational Interviewing (MI), Harm Reduction, and Housing First modalities.
- Additional Duties
- Undertake additional responsibilities as assigned.
