Job description

Position: PACE Registered Nurse (RN)

Overview: The PACE RN will perform primary care duties while focusing on annual wellness checks for PACE participants. Training will be provided, and shadowing opportunities are available in the field. On a typical day, the RN will attend a morning team meeting, participate in a huddle clinic, and see patients from 10 AM to 4 PM. Occasionally, the RN will conduct annual wellness checks in the afternoon. They will see approximately 5-15 patients per day.

Position Summary: As a Nurse in the NEIGHBORHOOD PACE Program, you will be part of a dynamic Interdisciplinary Team (IDT) and contribute to an innovative care model that supports older adults in staying healthy and independent. The PACE program has one of the longest-standing programs for elderly care nationwide, offering an opportunity to work in one of the largest community health centers in the country.

Essential Duties and Responsibilities:

  • Oversee, assess, plan, and deliver nursing care to PACE participants.
  • Assess patients by gathering histories, performing tests, and consulting with caregivers and IDT members.
  • Educate patients and families on health regimens, wellness, medications, diet, and more.
  • Safely administer medications, tests, and treatments; manage medication refills with provider approval.
  • Provide health education to individuals, families, and groups to help maintain optimal health.
  • Conduct biannual and status change nursing assessments for an assigned panel of participants, documenting care plans.
  • Utilize assessment tools like PACE Nurse Assessment and Medication Management to collect data.

Qualifications and Requirements:

  • Baccalaureate degree in nursing preferred.
  • Current Massachusetts nursing license required.
  • Current BLS certification required.
  • Minimum of 2 years of experience in ambulatory, home health, hospital, or sub-acute settings preferred (equivalent training may be considered).
  • At least one year of experience working with frail or elderly populations.
  • Experience in an interdisciplinary team setting is a plus.
