HYBRID Infectious Disease Nurse Case Manager

The RN Case Manager will be supporting HIV and TB clinics as well as Hep B and C clinics as needed, carrying a caseload (60-70 patients) and they would be supporting 1 Dr. who has two sessions/week. For the TB clinic they would be working in a state funded TB clinic (first in the U.S.) where they will be managing patients and helping to do treatments (vaccines/plans). This is a very independent job where the RN will make sure that patients get all proper scans and tests prior to visits. They RN will be doing a lot of the follow up appointments via phone and telehealth. They will be working hand in hand with IV labs, patients and the state.

The RN Case Manager for Infectious Disease supports the provision of coordinated medical and integrated behavioral health services to patients within the Harm Reduction Program working with patients with HIV, TB (active and latent), Hepatitis B and Hepatitis C Virus (HCV), Sexually Transmitted Infections (STIs: Chlamydia, Gonorrhea, and Syphilis) and those who access Syringe Services.

Monday-Friday

• Flexible on four 10 hour shifts or five 8 hour shifts
• Can be hybrid (Monday/Fridays) but must be onsite Tuesday-Thursday
• Flexible on exact shift (start/end time) as the clinics are open all day but ideally wants the RN to be able to work later shift on Tuesdays (until 8pm)
