Schedule/Shift: Monday - Friday, 8:30 am - 5:00 pm RN position

Pay Rate: $40-60+/hour, depending on experience

Job Description:

The Registered Nurse will serve a dual role, encompassing case management responsibilities and direct clinical support. As part of the PACE program enrollment, patients receive a nurse-conducted physical assessment. The nurse will conduct regular home visits to assess patients and aid with their basic Activities of Daily Living (ADLs). While most patients live within walking distance of the clinic, a few reside in the following locations: West Seattle, Everett, Renton, Greenlake, Mercer Island.

The incoming Registered Nurse will have a dual role, handling both case management responsibilities and providing direct clinical support. As part of the PACE program enrollment process, patients undergo a physical assessment conducted by a nurse. In this role, the nurse can anticipate regular home visits to assess patients and assist with their basic Activities of Daily Living (ADLs). While most patients live within walking distance of the clinic, there are a few residing in the following locations:

  • West Seattle
  • Everett
  • Renton
  • Greenlake
  • Mercer Island

Duties and Responsibilities:

  • Perform nursing assessments, skilled nursing treatments, interventions, and medication administration for program clients, with detailed documentation.
  • Assess clients and residents to evaluate their self-medication management skills, medical stability, and congruity of care needs with medical diagnoses before their move-in.
  • Communicate client treatment and health-related issues to the team, physicians, clients, caregivers, State Case Managers, and community service providers.
  • Collaborate with other staff to monitor changes in resident/client condition and adjust services as required.
  • Assist in scheduling client/resident medical appointments when necessary.
  • Inform relevant parties about changes in client/resident health and ensure documentation of changes in health status.
  • Oversee and maintain the medication assistance program through staff training.
  • Manage the purchasing of health-related items and inventory control within budgetary guidelines.
  • Contribute to providing and maintaining a safe environment for residents/clients and staff.
  • Ensure correct processing and documentation of medication changes.
  • Review monthly new Medication Administration Records (MARs) and medication refills for accuracy.
  • Ensure adherence to RN delegation policy and procedures when providing RN delegation to resident services specialists.

Job Requirements:

  • BLS Certification.
  • Registered Nurse Certification through Washington State DOH.
  • Experience working in Community Health or Geriatrics (preferred).
  • Case Management experience (preferred).
