Job description

The RN Case Manager will work with providers, team nurses, and care/case managers to ensure that the patients receive patient assessment, care planning, education, follow-up, and support in developing and achieving their health-related self-management goals and best health outcomes. The RNCM collects, maintains, reports, and follows up on related clinical data. She/he assists in developing day-to-day management and long-term planning for the patient's care. The RNCM collaborates with health center providers and multidisciplinary professionals to provide physical, psychological and/or social support for discharged patients and families. She/he identifies and establishes relationships with community resources that can contribute to the network of care for patients.

Overview:

  1. Coordinating with inpatient social work and nurse case management to enhance discharge planning and post-discharge appointments.
  2. Meeting with the patient's clinical team for discharge planning.
  3. Meeting with admitted patients (established or intended) to build engagement in care.
  4. Mornings will be spent at the hospital and afternoons spent at the health center - Mileage between the two locations will be reimbursed at 70 cents per mile.
Must Have Skills/Qualifications:
  • Up to date RN license in the state of Massachusetts
  • At least 2 years of Case Management experience
    • No particular setting required
  • At least an Associates degree
Schedule/Shift: M - F 7:00am or 7:30am - 3:00pm or 3:30pm. No weekends or holidays.
