Job description

Job Title: RN for Nurse Family Partnership Program (Medicaid program)

Pay: 90k (not negotiable and will get minimum 3% cost of living every year / great vacation package / 14 paid holidays / mileage reimbursed, 401k)

Start Date: ASAP

Schedule/Hours: 35 hours M-F 9-5

Training Process: training is around 4-6 months (hybrid, 2-3 days in office and 3 days in the field)

Location: field work in Staten Island

Caseload: 25

Language Requirement: Spanish/fluent

Requirements

  • Education: BSN
  • Bilingual: Fluent in Spanish required
  • Previous Industry Experience: women's health, behavioral health, pysch, home care, maternity health
  • Open to New grads

Day to Day Overview

  • Some flexibility to meet the needs of the client in general 9-5 or can be more flexible

eventually they start to schedule their own clients

  • no more than 12-15 visits per week
  • 1 hour supervision call with supervisor
  • Wednesday 10-12 team meeting
  • Services are in person (there is telehealth component but it is to supplement in home visits)
  • The first year is a slow build of caseload max is 25 usually between 21-25 - will be doing a ton of training in first year - that is on zoom live sessions with camera on and request to come into the office space for that so they have someone to help for tech difficulties or knowledge gaps
  • Shadow visits to become familiar with the model
  • Very relationship based role
  • Home care, psych, new grads, labor and delivery experience preferable
  • Tribeca the first week to pick up equipment (travel quarterly)
  • Home office in Corona/Queens
  • Need a car and active driver license

Position Description:

The Nurse Family Partnership (NFP), a program of Public Health Solutions, is a nationally recognized, evidence-based nurse home-visiting program for high risk, low-income, first-time mothers, their infants, and their families. NFP seeks a Nurse Home Visitor to provide services throughout Northern Queens for this community based, collaborative, nurse home visiting program. You will assess the medical and social needs of pregnant women and infants; follow a prescribed process/plan of care and monitor the outcomes in accordance with NFP and Public Health Solutions' policies and procedures. Nurse home visitors report to the NFP Nurse Supervisor.

Specifically, the Nurse Home Visitor, R.N will:

  • Provide home visits to pregnant/parenting women and their families eligible for the NFP Program.
  • Provide education and case management services to women from pregnancy through the child's second birthday.
  • Adhere to nursing process and the NFP model of home visitation.

Clinical Duties:

  • Develop therapeutic relationships, utilizes concepts of reflection and motivational interviewing with women and their families in a home visiting environment.
  • Perform home visiting in accordance with the NFP model and guidelines.
  • Follow nursing process in implementing NFP into nursing practice.
  • Assess physical, emotional, social and environmental needs of women and their families as they relate to the NFP domains.
  • Assist women and their families in establishing goals and outcomes.
  • Provide education, support and referral resources in assisting women and their families in attaining their targeted goals.
  • Consult and collaborates with other professionals involved in providing services to women and families.
  • Refer families for necessary services and following up to ensure that families are receiving services.
  • Complete all documentation required for Medicaid billing.
  • Perform related duties as assigned or required.
