Job Title: RN Care Manager (DC licensed RN)

Hybrid role: Mix of field work 3x/week and remote

Schedule: Mon-Fri, standard hours - will manage your own schedule (home visits, doc appoints, public places for field visits)

Skills/Qualifications:

2+ years of case management

Ambulatory experience, Health plan experience, Pediatric experience (preferred)

Pay: $42-46/hr

Typical Case Management - will be taking care of enrollees in DC Medicaid - much of whom also have behavioral health issues and poor social determinants of health

Care Managers will start off each patient with needs assessment - identify where they are at, assign them acuity level between 1-3. This will decide what level case manager they will be assigned to and how frequently the Care Manager will need to be in contact with them.

1 = fairly well managed, don't need to use a lot of medicaid $
2= Use a moderate number of Medicaid dollars, will need more attention than level 1.
3 = not well managed, high utilizers - will need more touchpoints per month
