Job description

The setting for this RN Case Manager outpatient care environment, where the

focus is on managing patient care outside of a hospital. The role involves both
in-person and remote care management, coordinating with care teams, and
overseeing discharge planning. This position provides ongoing care for patients
post-hospitalization or those managing chronic conditions. The inclusion of
telehealth and wound care also hints at a focus on transitional care and long-term
patient management.

Work Location: On-site (1 remote day possible - hybrid model)


Responsibilities:
● Assess and prioritize patient care needs during initial visits.
● Collaborate with interdisciplinary care teams to coordinate and implement
patient care plans.
● Monitor high-risk patients, providing coaching for self-management and
ongoing follow-up.
● Manage discharge planning, including home health and support services.
● Oversee a caseload of up to 75 patients, ensuring smooth care transitions.
● Participate in daily huddles and relevant meetings.
● Provide wound care and treatment as needed.
● Utilize telehealth services for remote patient assessment and management.
● Support patients post-hospital discharge to ensure recovery.


Must-Have Skills/Qualifications:
● Active Washington State Registered Nurse license.
● Basic Life Support (BLS) certification (AHA) or CPR/AED certification (American
Red Cross).
● Minimum of 3 years of experience in a clinical setting.

Nice-to-Have Skills:
● Experience in geriatrics or outpatient triage.
● Knowledge of Medicare/Medicaid.
● Ability to work independently.
● Experience with electronic medical records (EMR), preferably EPIC.

Schedule:
● Monday to Friday, 7:30 AM - 4:00 PM (40 hours per week).

Contract Length:
800 Hours. Opportunity to convert to permanent hire after satisfactory completion of
contract.
