Essential Duties and Responsibilities:

As a RN Case Manager you will…

• Conduct patient assessments and develop individualized care plans.
• Educate patients and families about CKD and treatment options.
• Coordinate care with healthcare providers and facilitate referrals.
• Advocate for patients and assist in navigating healthcare services.Monitor patient progress and adjust care plans as needed.
• Maintain accurate patient records and ensure compliance with regulations.

Supervisory Responsibilities: This position has no supervisory responsibilities.

Experience/Requirements

• Bachelor's Degree preferred.
• CCM (certified case manager)
• Registered Nurse (RN). Must have current unrestricted nursing licensure in practicing State with ability to obtain licensure in multiple states.
• Experience working in NCQA Accredited Org.

Nice to Have Skills:

• Medicare Advantage experience
• Bilingual in Spanish
• Experience with ESRD and/or dialysis
• Familiarity with Medicare Guidelines and MCG

Soft Skill/Attribute Requirements:

• Capable of priority setting
• Excellent analytical skills
• Must be organized and detail oriented with a strong bias for follow-up and problem resolution
• Proven ability to be a team player.

Computer Skills: To perform this job successfully, an individual should have knowledge of basic office equipment such as: "EMR (electronic medical record system) MCG, Microsoft Excel computer, internet, email, copy and fax machines, and telephone.

Work Environment: This position is going to be FULLY REMOTE.
