Location: Washington, DC

Schedule: Monday to Friday, 8:00 AM to 5:00 PM

Job Type: Full-Time, In-Person

Salary Range: $85,000.00 - $96,000.00 per year (Based on nursing experience)

Overview: We are seeking a dedicated and experienced Pediatric Discharge planner to join our dynamic healthcare team. This position offers a unique opportunity to work in a collaborative environment, coordinating care for pediatric patients with various diagnoses. The ideal candidate will have a strong clinical background and preferably experience in case management, along with exceptional communication and organizational skills.

Responsibilities:

  • Coordinate care for pediatric patients, including discharge planning and ongoing case management.
  • Work collaboratively with a multidisciplinary team including providers, utilization management, nutrition, and social work.
  • Conduct daily rounds at a specific time (usually around 10:30 AM) with the same team each day.
  • Cover up to 25 beds per day, although it's rare to reach this number.
  • Participate in a 6-8 week orientation/training program, with additional shadowing and weekly check-ins for the first two months.
  • Advocate for patients' needs and ensure appropriate care planning.

Requirements:

Must-Have Skills/Qualifications:

  • Registered Nurse (RN) with a Bachelor of Science in Nursing (BSN)
  • Strong clinical background as a nurse
  • Confidence to fulfill the role effectively
  • Ability to work well with a team and advocate for what is right
  • Exceptional communication and organizational skills
  • Familiarity with Cerner or similar electronic health record systems

Nice-to-Have Skills:

  • Master of Science in Nursing (MSN)
  • Certified Case Manager (CCM) credential
  • Previous experience in case management

Experience:

  • Prior experience in pediatric care is highly preferred
  • Previous case management experience is advantageous

Certifications Needed:

  • RN, BSN
