Job description

Medix is currently hiring for a RN Care Manager in one the top hospital in the Chicagoland area. This position will be fully onsite (Monday - Friday). The RN Care Manager is responsible in facilitating patient care by determining patient needs, evaluating treatment options, creating treatment plans, coordinating care, assessing progress while practicing stewardship. The RN Care manager will work proactively with physicians, residents, social workers, and families as it relates to discharge planning.

Schedule: Monday - Friday (rotating weekends)

JOB DUTIES

  • Demonstrates the WMH Customer Service Values, which are key in providing quality service to patients and customers
  • Attends interdisciplinary rounds daily
  • Educating patients about their care plan and helping them make key decisions
  • Appropriately document and update discharge plans in EMR
  • Establishes and maintains criteria for identification and resolution of problems associated with the Case Management/Utilization Review
  • Coordinates the overall discharge planning and social services functions, ensuring compliance with federal and state laws and the TJC
  • Maintains a liaison with community agencies offering services to discharged patients
  • Performs additional assignments and tasks as assigned by management.

Requirements

  1. Must possess a Bachelor's degree from an accredited school of nursing.
  2. Current Illinois Registered Nurse License required.
  3. CPR Certification
  4. Ability to establish and maintain effective working relationships with the public.
  5. Must be able to follow directions and to perform work according to department standards when no directions are given.
  6. Must be emotionally mature and able to function effectively under stress.
  7. Must possess critical thinking skills, effective written and oral communication skills, strong self-initiative and interpersonal skills.
  8. Must be available to rotate weekends and holidays including hospital recognized holidays.
